I'd misapplied a patch. It works now. Here's exactly what I did:

1) export CC=gcc CFLAGS="-O2 -mlp64" LDFLAGS=-mlp64

2) Applied attached patch.

3) ./configure --without-readline --without-zlib \
--prefix=/opt/pgsql/postgresql-7.4.5-64bit
