My PostgreSQL version is 7.4.1, and I performed a Reindex and the problem still remains.  
  
due to our system configuration, we cannot Upgrade it. (cluster environment).

You've got a corrupted index --- try REINDEXing that table.  And
consider updating to a newer Postgres, if it's less than 7.4.15.
